Cancellation Agreement *

In accordance with the membership contract, I understand that cancellation requests must be submitted no later than 14 calendar days before my preferred membership end date. I understand that all remaining debit(s) within my 14 calendar day cancellation notice period will be processed as a lump sum at the time of my membership cancellation being processed. I understand that if I request my cancellation during my membership hold/freeze, my remaining hold/freeze will cease and my membership will enter the 14 calendar day cancellation notice period as stipulated above.
